Hospice at Home Service

 

We are a community based service that can provide Specialist Palliative Care to adults and children living in Cos. Limerick, Clare and North Tipperary.  Our aim is to support people who choose to be cared for, and sometimes die, in their own home or in the local community hospital or nursing home.

We bring the specialist treatment, advice and support that you would receive in our Specialist In-patient Unit to you.

We are consultant led, multi-disciplinary team consisting of:

Palliative Medical Team

Nursing staff

Social work and counselling   

Pastoral care

Physiotherapy

Speech and Language Therapy

Occupational Therapy

We work from bases located in Ennis, Limerick, Nenagh, Newcastle West and Thurles. Our service works in close collaboration with:

• Your GP, Public Health Nurse and other community services
• Your Hospital Consultant
• Other agencies that may be providing care for you
